Clinical Health Director of Hospice.
Cook County Hospital is currently seeking a Hospice Director to join a team focused on exceptional service to patients and employees alike.
Duties:
Director will work FT- Monday- Friday.
Forecast needs, set priorities and execute business strategies.
Make calls to referrals each day to promote hospice services.
Communicate effectively with all members of the health care delivery team.
Provide exceptional care to hospice patients and receive updates as needed.
Will help conduct surveys within home health and needs to have a knowledge of the Phoenix area.
Requirements:
Registered Nurse with a current state license
Ability to participate in Medicare/Medicaid programs
Current CPR certification
CHPN certification, preferred
Healthcare leadership experience- two (2) years hospice, preferred
Experience in providing services to terminally ill patients
Access to a reliable automobile 24 hours a day and meets state and company regulations regarding automobile liability insurance coverage.
Valid Driver's License
Capable of maintaining regular attendance
